**In an effort to update our records, this is to advise you that we require an Insurance Certificate from you as the monument Dealer and one from the Setter who installs the monuments on our premises. One copy of the Certificate should be sent to us covering Maimonides Cemetery at 90 Elmont Road, Elmont, NY 11003 and Maimonides Cemetery at 895 Jamaica Avenue, Brooklyn, NY 11208. Alternatively and preferably send to [email protected].

FOR FUNERAL DIRECTORS
Burial arrangements must be initiated by the funeral director. The funeral director can call the cemetery any time to make arrangements. After hours calls will be returned quickly. The cemetery will coordinate time of burial with the funeral director. The lot will be inspected and the grave opening fee and allowable casket size will be determined. All graves are opened by hand so reasonable time is needed to accomplish this. The following information is needed:
Name of deceased, location of grave, date of death, place of death, place of birth, age, next of kin. We will discuss allowable casket size, which depends on the lot.
Rates below. Call for quote specific to burial arrangement and lot. Chairs and tent available.

Maimonides Cemetery offers burial plots for sale in our Elmont Cemetery at $6500 per grave. Beit Chaverim, the interfaith burial ground in Elmont is offered at the same price.
Roadside and family plots are also available. Price upon request.
